Forms, Registration, Assessment and Disclosures

One particular question, problem that you have to solve, or form come to mind?

Like: ” Are you vaccinated” or “When  or with what were you vaccinated”,

or, here is a fun one… “Why are you not vaccinated?” 

We hope you are not still relying on a pdf forms or some type of static system.

Yikes, sorry, you probably are.

So, let’s start with the idea that having your users fill out a form should actually save you time and effort, not cost you time and effort.

That you probably have more than one form.

That you probably need a group of forms to be filled out, annually?

That every year the person/s who is in charge of this task has to be convinced again, or a new person has to be tricked into do it.

That tracking these forms and helping the user fill “ALL” of it out is a ridiculously mundane and massive task.

And that at the end of the day…

You probably need someone to file them all, input in all the information, and well, find out everyone who did not do them, or worse, try and find out who answered a specific question a specific way.

Do you have specific form might be required for a specific group?  Do you have hundreds of groups? How do you get that done, properly? How do you comply? How do you audit? I know we are not supposed to ask that question.,

So, just that one task, just one or two forms can take an army of people to make sure they get done, make sure they are done properly, make sure the information is properly input and then the forms are stored so that they can be retrieved.

Just one guess how you are managing today… “Paper (or pdf (it’s the same)) and an excel spreadsheet.”

That sounds fun, and lets not mention if any of it is really useful at the end of the day.

Our system does it all and at the end of the day you have a record of compliance, and you have all the data you asked for.

And it was quick and easy.

We created a tool for making forms, just not the forms.

Because believe it or not, everyone’s forms are different.

So, you ask a question like: “Are you Vaccinated” (I know that’s a loaded one).

Let’s call this the Covid Vaccine Registration form (this is one that everyone is dealing with.)

And let’s say the answers are:

1.  ANSWER: Yes, Fully
   • Let’s take that one and make a “Vaccinated Flag” for the user’s ID and reporting.
   • Let’s also make a Vaccinated Wallet Pass.
   • Let’s also make a section in the form where the user can upload their vaccine pass.
   • Let’s also run a report so that we can track all the people who are vaccinated.
   • Let’s also create a field in the users health area that will track the person’s vaccine status and update it.
2. ANSWER: Not Yet, need one more shot!
   • Let’s take that one and make a”Partially Vaccinated Flag” for the user’s ID and reporting
   • Let’s also make a “Partially Vaccinated Wallet Pass”.
   • Let’s also run a report so that we can track all the people who are partially vaccinated.
   • Let’s update the users vaccine status field in the users health area.
   • Let’s make a question in the form that asks, if partially vaccinated when will you receive your second shot?
   • Let’s send out a follow-up message to all the partially vaccinated and ask that they update the form when they get their second vaccine. We can use the date of the second vaccine to ask them at the time they have.
   • Let’s allow a user to update the form when they are fully vaccinated so that we can capture all the updated relevant information.
3. ANSWER: No, Medical or Religious Exemption.
   • Let’s take that one and make a” Vaccine Exemption Flag” for the user’s ID and reporting.
   • Let’s also make a “Vaccine Exemption Wallet Pass”.
   • Let’s create an area in the form so that the user can upload the exemption letter.
   • Let’s also run a report so that we can track all the people who are not vaccinated due to an exemption.
   • Let’s update the users vaccine status field in the users health area.
   • Let’s make a form that implements a testing form that the user can update based upon our requirements for users who are not vaccinated.
4. ANSWER: No, and I am not going to!
   • Let’s send them a message via email, text or secure messaging and explain what the implications are of not getting vaccinated.
   • Let’s also run a report so that we can track all the people who are not vaccinated.
   • Let’s update the users vaccine status field in the users health area.
   • Let’s create an “Unvaccinated Flag” and a wallet pass and identify the user as unvaccinated.
5. ANSWER: Not right now because I am sick with COVID
   • Let’s send them a message via email, text or secure messaging and explain what they need to do if they are infected.
   • Let’s also run a report so that we can track all the people who are infected.
   • Let’s create an “Infected Flag” and update the user’s profile with the flag.
   • We can issue an “Covid Infected User Wallet Pass” that expires in 10 days.
   • Let’s update the users covid infection status field in the users health area.

Tracking completion, managing different “answers”, and taking specific steps (like flagging users or creating wallet passes takes a form and a user’s answer and gives it POWER!

So, now we can ask on the same form. Who answered, Not Vaccinated”. Let’s send them a message. So, now we need to ask all of those who have not completed the form to complete it, so let’s send them an email, text, or secure message telling them we need them to complete it.

We are having a pizza party for everyone who is vaccinated, let’s send them a message.

For the pizza party, let’s make sure that everyone has downloaded their “Vaccinated Wallet Pass”, so they can show it to us, to enter the pizza party!

21. Administrators can go back and see any completed form at any time and see a history by user.
